Job summary

Option Care Health, Inc. is seeking a Clinical Transition Educator (RN) in Cleveland, OH to educate patients, caregivers, and facility staff on transitioning to our home and alternate-site infusion model.

You will collaborate with the sales team to identify educational needs, perform patient assessments, and coordinate smooth hospital-to-home transitions, ensuring high-quality, compliant care across care settings. Must hold an active RN license and be prepared for local travel.

Qualifications

• Active and unrestricted RN license required.
• Experience in healthcare preferred.
• Strong customer service and communication skills required.

Responsibilities

• Educate patients, caregivers, and facility staff on OCH services and home/in-home care.
• Conduct patient assessments to determine eligibility for the care model.
