Job summary

Option Care Health, Inc. seeks a Clinical Transition Specialist to sell services to discharging patients, educate patients and families, and coordinate care transitions to home or an alternate site. You will build strong relationships with referral sources and collaborate with Account Managers to meet sales goals.

Required are an active RN or Pharmacist license and at least 2 years in healthcare, plus strong relationship-building and presentation skills. Travel to care facilities is involved.

Qualifications

• Licensed RN or Pharmacist in the practicing state with at least 2 years in healthcare.
• Experience establishing and maintaining relationships with individuals at all levels of the organization and with vendors.
• Knowledge of ACHC, URAC standards and related practices.
• Proficient in basic PC tools and office software (Word/Excel/PowerPoint).

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Educates patients, families, and facility staff about Option Care services and products.
• Coordinate with internal partners and external case managers to ensure smooth patient transitions home or to ATS.
• Collaborates with Account Manager and Regional Sales Director to execute area growth plans and report progress.
