Job summary

Option Care Health Inc. is a leading home infusion provider seeking a Clinical Transition Specialist to educate patients and caregivers about options for care at home or alternate sites.

This role focuses on developing strong relationships with discharge planners, case managers and hospital staff to facilitate timely patient transitions and service adoption. The position requires an RN or Pharmacist license with at least 2 years in healthcare, and travel to partner facilities and hospitals to

Qualifications

• Licensed RN or Pharmacist in the state of practice.
• Minimum 2 years of healthcare industry experience.
• Experience establishing relationships with diverse stakeholders and providers.

Responsibilities

• Proactively grows relationships with referral sources to increase sales and patient starts.
• Serves as primary point of contact for referrals and education of staff.
• Educates patients, families, and facility staff about services and products offered by Option Care.
• Coordinate with Account Managers to execute area growth plans and improve referral rates.
• Ensure accurate collection and sign-off of pre-admission information.
