Job summary

Option Care Health is seeking a Clinical Transition Specialist (RN) to educate patients and families about home infusion services and coordinate discharges to alternative sites. You will build strong relationships with hospital staff, discharge planners and referral sources while ensuring a smooth transition to home care.

The role requires clinical expertise as an RN, excellent communication, and travel to care facilities.

Qualifications

• Licensed Registered Nurse (RN) in the state of practice with at least 2 years in healthcare.
• Proven ability to establish and maintain relationships with individuals at all levels of an organization.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Educates patients, families and facility staff about services and products offered by Option Care.
• Coordinate care transitions to home or alternate site and ensure timely discharge and therapy independence.
