Job summary

Wellstar Cobb Hospital in Georgia is seeking a Care Coordinator RN to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure discharge planning and efficient care progression.

The CC RN will work with physicians and the care team, participate in interdisciplinary rounds, and support transitions from hospital to next level of care, with emphasis on timely disposition and resource optimization.

Qualifications

• Associates Nursing or Diploma required.
• RN license required; BLS certification.
• Experience in acute care setting preferred.

Responsibilities

• Assess transitional care needs and plan discharge.
• Coordinate care with physicians and care team.
• Educate patient and family; participate in interdisciplinary rounds.
