Job summary

Wellstar Cobb Hospital in Georgia is seeking a Care Coordination RN to assess transitional care needs, coordinate across the continuum, and engage patients and families to ensure care goals are met. You will plan discharge, manage length of stay, and optimize resource use while collaborating with physicians and the care team.

Responsibilities include psychosocial assessment, discharge planning, and post-acute referrals.

• Strong assessment, interview and organizational skills.
• Knowledge of local, state, and federal regulations.
• Ability to collaborate with physicians, care team and patient/family.

Responsibilities

• Assess transitional care needs and plan discharge.
• Coordinate care progression and post-acute referrals.
• Participate in interdisciplinary rounds and family education.
• Document care plans and ensure timely discharge.
• Identify barriers to discharge and liaise with care team.
