Job summary

The Care Coordinator RN (CC RN) at Wellstar Cobb Hospital is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure needs are met. The role focuses on discharge planning and efficient resource use.

You will collaborate with physicians, case managers, and the care team, participate in interdisciplinary rounds, and document care progression in the medical record while supporting patients through transitions.

Qualifications

• Associates degree or Diploma in Nursing; RN license required.
• Minimum 1 year nursing experience in acute care setting.
• Excellent written and verbal communication skills; able to work under pressure.

Responsibilities

• Assess transitional care needs and plan discharge.
• Coordinate care with physicians, nurses, and care team.
• Document care progression in medical records and coordinate post-acute referrals.
• Facilitate patient/family education and interdisciplinary rounds.
