Job summary

Wollborg Michelson Recruiting seeks a compassionate RN to provide in-person and virtual support for specialists and diagnostic services in Phoenix, AZ. You will educate patients, coordinate referrals, schedule appointments, follow up, and advocate for patient needs while helping reduce costs and improve outcomes.

Qualifications

• Bachelor's degree in Nursing is required.
• Active, unrestricted Registered Nurse license required.
• Current CPR certification.
• Experience with electronic medical records; no specific system experience required.
• Customer service or patient support experience.
• Case management or care management experience preferred.
• Ability to communicate effectively with patients, care teams, and external partners.
• Ability to protect confidential information at all times.

Responsibilities

• Provide in-person and virtual support through real-time consultation, patient education, referral coordination, appointment scheduling, and follow-up for specialist and diagnostic services.
• Manage the referral and specialist service process from beginning to end.
• Lead quality programs, including patient outreach related to care gaps and clinical needs.
• Support patients with chronic conditions through education, reminders, and ongoing assistance.
• Manage and integrate office processes.
• Submit service and appointment requests on behalf of patients.
• Provide the care team with referral information and requirements.
• Build relationships with healthcare providers and community organizations.
