Qualifications

• Active unrestricted RN license to practice.
• BLS certification within 30 days of hire.
• Experience with Electronic Medicare Records.
• Customer service orientation and experience.
• Strong communication across multiple modalities.
• Highly collaborative and empathetic.
• Must maintain confidentiality at all times.
• Fingerprint clearance within 30 days of hire.
• Immunization documentation or immunity proof required.

Responsibilities

• Support members in-person and virtually with real-time consultation, education, scheduling of referrals, and follow-up.
• Contribute to improved health outcomes and cost savings.
• Serve as a visible partner to patients and care team.
• Explain referral and diagnostic service recommendations clearly.
• Manage end-to-end processes for referrals or specialist services.
• Lead development of quality programs and patient outreach.
• Assist members with chronic conditions through education and reminders.
