Highmark Health is seeking an RN Nurse Navigator to serve as a consistent contact for referring physicians, patients, caregivers, families, and the community. The role facilitates patient flow and coordinates care for adults within specialized disease states in Pennsylvania.

The position involves outreach, education on diagnoses and treatment options, safe handoffs between facilities, and collaboration with High-Risk Care Teams to reduce readmissions.

Qualifikationen

• Bachelor's degree in Nursing preferred.
• 1-3 years nursing experience in the specialty required.
• Current State of PA RN licensure or multi-state licensure via eNLC.
• CPR – American Heart Association.
• Act 34 Criminal Background Clearance Certificate.
• Act 33 Child Abuse Clearance Certificate.
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
• Valid PA driver's license may be required.

Aufgaben

• Oversee community outreach and establish working relationships within the network.
• Educate and coordinate care regarding patients' diagnosis and treatment options.
• Ensure safe handoff and coordination of care between facilities.
• Work with High-Risk Care Teams to reduce readmissions.
• Assist Registry staff with data collection and patient outcomes.
• Coordinate appointments with the multi-disciplinary team.
• Address medication adherence issues.
• Identify at-risk patients and facilitate appropriate screening processes.
• Trend data and outcomes for the navigation program.
• Develop or attend clinical care conferences to report on active patients.
