Job summary

Winslow Indian Health Care Center is seeking an experienced Clinical Care Coordinator (RN) to join the Winslow, AZ facility. You will work within the Patient Centered Medical Home (PCMH) framework to coordinate care, review charts, and monitor tests across multidisciplinary teams while upholding confidentiality and care standards.

Qualifications

• Registered Nurse license in good standing in any U.S. state.
• Bachelor’s degree or higher in Nursing; ADN with BSN completion within four years will be considered.
• Navajo language skills preferred.
• Must have a valid driver’s license and pass a background check.

Responsibilities

• Performs chart reviews for upcoming appointments in their panel.
• Tracks outstanding tests and results and monitors consult results.
• Conducts post-discharge follow-up calls for high-risk patients.
• Coordinates care between hospitals and clinics; maintains care continuity databases.
• Collaborates with multidisciplinary teams to maximize patient health outcomes.
• Manages panel care, health maintenance, and adherence to AAACN principles.
• Ensures confidentiality and HIPAA compliance in all duties.
