Job summary

Wollborg Michelson Recruiting in Phoenix, AZ seeks a Registered Nurse to provide in-person and virtual support through real-time consultation, patient education, referral coordination, appointment scheduling, and follow-up for specialist and diagnostic services.

You will collaborate with the care team to manage referrals from start to finish, support patients with chronic conditions, and maintain EMR records while building strong relationships with providers and community partners.

Qualifications

• Bachelor's degree in Nursing is required.
• Active, unrestricted RN license required.
• Current CPR certification required.
• Experience supporting patients, coordinating referrals, and managing clinical services.
• Ability to protect confidential information at all times.

Requirements

• Current license to practice as a Registered Nurse.
• Current CPR certification.
• Experience with electronic medical records;no specific system experience required.
• Customer service or patient support experience.
• Case management or care management experience preferred.
• Ability to communicate effectively with patients, care teams, and external partners.
• Ability to protect confidential information at all times.
