Job summary

LiveWell Homecare Agency, LLC is seeking an experienced RN Healthcare Manager to support high-risk patients through assessment, education, and advocacy, aiming to improve quality of life.

You will identify high-risk patients, coordinate care plans, perform clinical tasks within licensure, and document in electronic medical records. Must have RN license, BLS, and 4+ years in clinical settings; bilingual abilities a plus.

Qualifications

• Bachelor’s or Associate’s degree in Nursing with 4+ years of experience in clinical or hospital settings.
• Active RN license, current BLS.
• Knowledge of chronic diseases such as COPD, asthma, diabetes, CHF, and IHD.
• Proficient in electronic medical records (EMR).

Responsibilities

• Identify high-risk patients and manage chronic conditions through assessments, medication reconciliation, motivational interviewing, and referrals. Facilitate transitions from inpatient to outpatient care and coordinate care plans.
