Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager to support high-risk patients and help them navigate the care system in Mesquite, TX. This RN role collaborates with physicians, nurses, pharmacists, social workers, and other educators to educate and advocate for patients and families, aiming for maximum quality of life.

The position emphasizes transition of care, patient education, and care coordination in a less-structured environment.

Qualifications

• RN with active and unrestricted license in state of employment.
• Current BLS certification.
• 4+ years of RN experience in a physician’s office, clinic, or hospital setting.
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD).
• Proficient with electronic medical records (EMR).
• Excellent verbal and written communication; strong interpersonal skills.
• Ability to work with multidisciplinary teams and coordinate care.
• Must meet tuberculosis screening and immunity/vaccination requirements.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care and patient education.
• Coordinate transitions from inpatient to outpatient settings and update care plans.
• Document accurately in EMR and participate in PCC meetings and weekly reports.
• Maintain evidence-based nursing practices and perform duties within RN scope.
