Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager (RN) in Texas to navigate care for high-risk patients. You’ll educate, advocate, and coordinate with physicians, nurses, pharmacists, and social workers to maximize quality of life in a supportive, autonomous setting.

The role emphasizes patient education, transitions of care, and data-driven care coordination. A BSN/ADN with RN license and BLS are required; 2+ years in clinical settings is expected.

Qualifications

• Bachelor’s or Associate degree in Nursing with RN experience
• Active RN license in the state of employment
• Current BLS certification
• 2+ years in a physician’s office, clinical or hospital setting
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD)
• Proficient EMR skills
• Excellent verbal and written communication
• Strong interpersonal and teamwork abilities
• Organizational and prioritization skills

Responsibilities

• Identify high-risk patients with providers and clinic staff
• Support longitudinal care: assess health, reconcile meds, MI, self-management goals, patient education, referrals
• Assist with transitions from inpatient to outpatient care and contingency planning
• Coordinate with providers to update the care plan
• Document accurately in the EMR
• Participate in daily huddles and PCC meetings
• Prepare reports for weekly meetings
• Maintain competency and stay current with evidence-based practices
• Perform other duties within scope as assigned
