Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager to support patients with high-risk health conditions and help them navigate the healthcare system to improve their quality of life.

You will collaborate with multidisciplinary care teams, including physicians, nurses, pharmacists, and social workers, acting as an educator, resource, and advocate. The role includes nursing duties within the RN scope and coordinating care transitions.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with four or more years of RN experience.
• Active and unrestricted RN license in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical or hospital setting.
• Knowledge of chronic diseases including COPD/asthma, diabetes, CHF and IHD.
• Proficient computer skills to use EMR systems effectively.
• Excellent verbal and written communication skills.
• Strong interpersonal and organizational skills.
• TB screening and immunity to listed diseases is required.
• Tuberculosis screening and immunity documentation as required

Responsibilities

• Identify patients at high risk with providers and clinic staff.
• Support longitudinal care: assess health, reconcile meds, MI and self-management goals, patient education, referrals.
• Assist in transitioning patients from inpatient to outpatient settings; assess needs and educate.
• Coordinate with providers to establish or update the plan of care.
• Perform accurate documentation in the electronic medical record.
• Participate in daily huddles and PCC meetings.
• Prepare timely reports for weekly meetings.
• Maintain competence and knowledge of evidence-based practices.
• May perform clinical tasks within scope and perform other duties as assigned.
