Job summary

WellMed, part of the Optum family of businesses, in Dallas, TX, seeks an RN Healthcare Coach to support high-risk patients and navigate the healthcare system. You will educate patients, coordinate care with a multidisciplinary team, and document in the EMR to improve quality of life.

Qualifications

• Bachelor's degree in Nursing or Associate's degree with 4+ years of experience in nursing.
• Registered Nurse with an active and unrestricted license to practice in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical or hospital setting.
• Experience interacting productively with individuals and with multidisciplinary teams.
• Proven knowledge of chronic diseases, COPD/asthma, diabetes, CHF and IHD.
• Proficient computer skills to work efficiently with electronic medical records.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care with assessments, medication reconciliation, and education.
• Coordinate transitions from inpatient to outpatient and ensure contingency plans.
• Document accurately and timely in the EMR; participate in PCC meetings.
