Job summary

WellMed, part of the Optum family, seeks a Healthcare Manager to support patients with high-risk conditions and navigate the healthcare system. You will educate, advocate, and coordinate with a multidisciplinary team to maximize quality of life.

You will conduct assessments, perform medication reconciliation, and assist with transitions from inpatient to outpatient care, while maintaining accurate EMR documentation and contributing to care plan development.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of RN experience.
• Active and unrestricted RN license in the state of employment.
• Current BLS certification.

Responsibilities

• Identify high-risk patients and support longitudinal chronic care management.
• Perform assessments, medication reconciliation, Motivational Interviewing, and patient education.
• Coordinate transitions from inpatient to outpatient care and support post-discharge needs.
• Maintain accurate documentation in the electronic medical record and participate in PCC meetings.
• Contribute to plan of care and prepare weekly reports.
