Qualifications

• RN license in Oregon and current practice compliance.
• Discharge planning and care coordination experience preferred.
• Strong communication and patient advocacy skills.

Responsibilities

• Completes and documents a discharge planning assessment.
• Facilitates development of a multidisciplinary discharge plan.
• Oversees and evaluates implementation of the discharge plan.
• Collaborates with care teams and post-acute providers for timely transitions.
• Identifies readmission risk and implements 30-day mitigation strategies.
• Responsible for patient education and advocacy.
