Virginia Mason Franciscan Health is seeking an RN Care Coordinator to lead discharge planning and care coordination across the continuum, advocating for patients and collaborating with physicians, nursing, insurers, and post-acute providers for seamless transitions.

The role emphasizes clinical acumen, superior communication, and strategic thinking to optimize patient care at every touchpoint within our Puget Sound network.

Qualifications

• Graduate of an accredited school of nursing
• Minimum two years of acute hospital clinical experience at hire
• Registered Nurse license for WA at hire
• Basic Life Support - CPR certification within 90 days
• BSN and 5+ years nursing experience at hire
• Certified Case Manager (CCM) preferred
• Accredited Case Manager preferred

Responsibilities

• Completes and documents a discharge planning assessment on patients identified by the screening process or upon request.
• Facilitates development of a multidisciplinary discharge plan engaging health team members, patient, and post-acute providers.
• Oversees and evaluates implementation of the discharge plan.
• Collaborates with the multidisciplinary team to ensure progression of care and appropriate inpatient utilization.
• Coordinates with post-acute providers to ensure timely transitions to appropriate post-acute services.
• Identifies readmission risk and implements interventions for a 30-day period.
