Job summary

Oregon Health & Science University (OHSU) is seeking an experienced RN Case Manager to oversee admission assessments, discharge planning, and care coordination within the Care Management department. The role collaborates with families, payors, and a multidisciplinary team to ensure safe, efficient transitions of care.

You will lead multidisciplinary rounds, coordinate post-acute needs, and contribute to reducing hospital length of stay and readmissions while upholding regulatory and CMS

Qualifications

• Minimum three (3) years of clinical nursing experience in the last 7 years.
• Minimum two (2) years of inpatient case/care management experience in the last 5 years.
• BSN from CCNE/ACEN/CNEA-accredited program 30 days before start date.
• ADN from an accredited program 30 days before start date.
• Current, unencumbered Oregon State Registered Nurse License.
• BLS from AHA required. New hires will be enrolled and required to complete during orientation.
• Must be able to perform the essential functions of the position with or without accommodation.

Responsibilities

• Assess admissions for family support, insurance resources, and discharge planning needs.
• Lead daily multidisciplinary rounds to develop discharge plans.
• Coordinate post-acute and discharge needs in a timely, cost-efficient manner.
• Collaborate with patient, family, and payors to determine appropriate level of care.
• Lead/or participate in family care conferences and ensure CMS/compliance with regulations.
• Support nurse case management in Emergency and Observation units.
