Somatus is seeking an RN Care Manager focused on VBC for CKD/ESKD populations. You will partner with physicians, health plans, and clinics to coordinate care, deliver in-home and clinic visits, and support high-need patients with complex needs.

You will travel to member homes, dialysis clinics, and offices, establishing trust with providers and patients while delivering education and care planning. A BSN or higher is preferred; travel is a core part of the role.

Qualifications

• RN license required and must be eligible to obtain licensure in target states.
• Minimum 2 years RN experience, with multi-disciplinary team involvement.
• BLS certification from a licensed facility or provider.
• Renal/Kidney disease or dialysis care experience preferred.
• Willingness to travel to member homes, clinics, and hospitals within assigned region.
• Strong understanding of HIPAA and patient privacy; compliant with regulations.
• Collaborates effectively with physicians, nurses, and care coordinators.

Responsabilités

• Perform home, clinic, and facility visits to support care coordination.
• Develop and update individualized care plans with the interdisciplinary team.
• Educate patients and families on discharge plans, medications, and self-management.
• Coordinate with nephrology practices to optimize workflows and outcomes.
• Document assessments and care activities in the EHR and CMS systems.
• Assist in scheduling and coordinating regular CM meetings with partner sites.
• Travel up to 75% of the time within the assigned market.
