Somatus in Phoenix, AZ is seeking an RN Care Manager, focusing on high-need CKD and ESKD patients, to coordinate home visits, dialysis clinic interactions, and provider collaborations.

You will lead care planning with interdisciplinary teams, educate patients on discharge and self-management, and travel within your region to support trusted nephrology practices. A valid RN license and BLS are required, with 2+ years of RN experience in multidisciplinary settings.

Qualificações

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience in multidisciplinary teams with physicians.
• BLS certification from a licensed facility is required.
• Renal, CKD or dialysis care experience as a main focus of your job.

Responsabilidades

• Outreach and scheduling visits to members in homes, dialysis clinics, and physician offices.
• Develop and implement individualized care plans with an interdisciplinary team.
• Educate patients on discharge requirements and self-management of renal conditions.
• Coordinate with nephrologists, PCPs and care team to improve patient outcomes.
• Travel within the assigned region up to 75% of the time to support care coordination.
• Lead care management coordination meetings and document in systems.
