Advocate Health in Illinois is seeking an experienced Registered Nurse with an active IL licensure to join our Care Management team in the Rolling Meadows area. You will collaborate with the patient/family and interdisciplinary teams to ensure delivery of high-quality, efficient care and to overcome barriers to timely treatment.

Qualifications

Requires IL RN licensure.
Bachelor's Degree in Nursing or related field.
Requires 5 years of experience in clinical nursing or 1-2 years of care management experience.

Responsibilities

Facilitates communications among patient/family and multidisciplinary teams to resolve barriers to timely care delivery.
Collaborates with care teams to provide quality, efficient, and cost-effective healthcare services.
Uses evidence-based approaches to engage patients and families in their own care and develop SMART goals.
Identifies barriers to learning and develops plans with the care team.
Independently manages caseload and ensures timely completion of tasks and documentation for MCO, regulatory, and contractual requirements.
Coordinates patient information and communications to ensure smooth transitions of care.
Coordinates referrals to internal and external departments and resources.
