Qualifications

• Active RN licensure required.

Responsibilities

• Conduct member assessments to identify LTSS needs.
• Provide care management and coordination for members with complex needs.
• Coordinate services across providers, community resources, and support systems.
• Monitor member progress and update care plans as needs change.
• Support transitions of care and follow-up on referrals and connections.
• Document assessments, care plans, and coordination activities.
