Requirements include a valid RN license, 2 years of nursing and 3 years of care management experience, plus certifications within 2 years.

Responsibilities

• Perform thorough patient assessments and develop care plans for discharge.
• Aggregate and report data on patient outcomes and resource use.
• Provide continuity of care and discharge planning for clinically complex patients.
• Lead mentorship and drive care-quality improvements.
• Advocate for patients to ensure timely care and resource use.
• Initiate referrals and document discharge planning activities.
• Coordinate with Social Work and multidisciplinary teams to ensure smooth transitions.
