Bon Secours Mercy Health is seeking a Manager of Care Management for St. Elizabeth Youngstown Hospital.

The role oversees daily operations of the Care Management Department, coordinating with leadership to standardize practices, optimize patient care, and support transitions across the continuum. The position requires a BSc in Nursing or Social Work, a valid RN or SW license, and a BLS certification, with a preference for a Master's degree and ACM/CCM/ANCC certification.

Formación

• BSc Nursing or Social Work required; master's preferred for some locations.
• BLS Basic Life Support certification required; RN license or SW license in state of practice.
• ACM/ CCM or ANCC Nursing Case Management board certification preferred.

Responsabilidades

• Provides leadership and oversees day-to-day operations to Care Management Team.
• Educates care management staff, physicians and nursing on progression of care and safe transitions.
• Utilizes systems to support proactive care management and effective transitions across continuum.
• Partners with physician leader to optimize utilization and provide data-driven opportunities.
• Participates in budget development and cost-reduction efforts.
