Mercy Health — St. Elizabeth Youngstown Hospital is seeking a Manager of Care Management to lead the daily operations of the Care Management Department and drive standardized practices across the continuum of care.

This role combines clinical oversight with financial stewardship to improve patient outcomes and resource utilization. The ideal candidate will have 3 years of acute hospital care management experience, a B.S.

Qualifications

• Bachelor of Science in Nursing or Social Work required.
• Master's degree in Nursing, Social Work, or Healthcare Administration preferred/required for specific facilities.
• 3 years of recent acute hospital care management experience is required.
• RN License or Social Worker license in state of practice is required, ACM/CCM/ANCC certification preferred.

Responsibilities

• Oversee day-to-day operations of the Care Management Team.
• Standardize practices related to care planning, LOS management, readmission prevention, and transitions.
• Collaborate with leadership to align utilization management and care coordination across the continuum.
• Direct budgeting and cost-reduction efforts within the Care Management Department.
• Partner with physician leaders to optimize committee operations and data-driven improvement.
