Job summary

Point32Health seeks a Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, coordination and navigation, and disease management per department guidelines.

The RN CM collaborates with members, caregivers, and providers to develop individualized plans of care, empowering patients for optimal wellness and self-management. Strong clinical knowledge and communication are essential.

Qualifications

• Registered Nurse with current unrestricted license in state of residence.
• Minimum 5 years’ relevant clinical experience.
• Bachelor’s degree required; nursing degree preferred.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment and assess needs.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions to next level of care and follow-up to prevent readmissions.
• Collaborate with interdisciplinary care teams to improve member outcomes and present high-risk members when appropriate.
• Maintain professional growth through self-directed learning.
