Job summary

Point32Health seeks an experienced Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum. You will coordinate transitions of care, manage complex cases, and support wellness interventions to empower member self-management.

The role requires strong clinical knowledge, critical thinking, and collaboration with members, caregivers, and providers to meet individualized goals. Remote work options may be available with standard office expectations.

Qualifications

• RN with current unrestricted license in state of residence
• Bachelor’s degree in nursing
• 5 years’ relevant clinical experience
• Experience in home care or case management
• Proficiency in a second language desirable

Responsibilities

• Perform telephonic member outreach and enrollments
• Assess members and identify needs and goals
• Develop and revise member care plans with care team
• Document in applicable platform per policy
• Provide health education and disease management support
• Coordinate transitions of care and follow-up
