Job summary

Habitat Health is seeking a Clinical Services Manager to lead nursing and clinic teams at a PACE center in California. Reporting to the Center Executive Director, you will oversee operations and guide RNs in case management while collaborating with the interdisciplinary team to deliver participant-centered care.

You will drive quality initiatives, ensure compliance with PACE/DHCS/CMS, and manage staffing, onboarding, and training.

Qualifications

• Active RN license in the state of practice.
• Three to five years of clinical leadership experience.
• Proven ability to supervise and develop clinical staff.
• Strong decision-making and prioritization skills.
• Advanced communication and relationship-building skills.
• Experience with complex geriatric populations.
• Knowledge of clinical quality standards and regulatory requirements.
• CPR/BLS certification.

Responsibilities

• Provide clinical leadership to IDT and clinic staff to ensure high-quality, participant-centered care.
• Ensure competency of direct reports through coaching, mentoring, and performance management.
• Lead onboarding, training, and continuing education for nursing and clinic team members.
• Monitor adherence to SLAs, regulatory timelines, and documentation requirements.
• Oversee care transitions and coordinate with Medical Director and center leaders.
• Drive KPIs through planning, workflow optimization, and quality initiatives.
• Maintain compliance with PACE, DHCS, and CMS regulations.
• Support clinic operations by addressing staffing needs and workflow issues.
• Provide direct clinical care as needed (triage, wound care, vaccines, home care).
• Perform other related duties as assigned.
