Job summary

Habitat Health is seeking an Enrollment RN to lead the clinical enrollment process for the PACE program, partnering with Outreach and IDT to conduct eligibility and home safety assessments. The role involves NFLOC assessments, education for participants and caregivers, and accurate EHR documentation, ensuring compliance with regulatory requirements.

Travel throughout the service area is required. The ideal candidate holds an RN license with at least one year of experience with older adults;

Qualifications

• Associate's degree in Nursing from an accredited program.
• Active, unrestricted RN license in the applicable state.
• Minimum of one year nursing experience with older adults or complex medical needs.
• Ability to conduct clinical/home safety assessments with sound clinical judgment.
• Strong organizational and communication skills; able to manage multiple priorities.
• Experience with EHRs and enrollment management systems.
• Willingness to travel throughout the service area.

Responsibilities

• Partner with Outreach Specialist and IDT to support a coordinated enrollment experience.
• Conduct comprehensive nursing, home safety, and eligibility assessments in participants' homes.
• Complete NFLOC assessments and DHCS documentation with timely follow-up.
• Present clinical findings at enrollment eligibility meetings.
• Perform home safety evaluations of prospective participants' homes.
• Educate participants and caregivers and address clinical questions.
• Maintain accurate clinical documentation in EHR and related systems.
• Stay updated on PACE eligibility, enrollment processes, and relevant regulations.
• Engage in outreach and community engagement as needed.
• Identify workflow improvements to enhance participant experience.
