Job summary

Habitat Health in California is seeking a Home Care Coordinator to ensure that personal and clinical homecare needs are delivered so participants thrive at home. You will be part of an Interdisciplinary Team (IDT) evaluating needs, developing care plans, and coordinating services to support safe, person-centered home care delivery.

The role requires at least 2 years of clinical experience in home care or geriatrics, strong communication, and proficiency with EHRs.

Qualifications

• Minimum of two (2) years of clinical experience, preferably in home care, geriatrics, care coordination, primary care, or a related healthcare setting.
• Knowledge of care planning, clinical assessments, and home care service delivery.
• Ability to conduct comprehensive participant assessments and identify changes in condition or care needs.
• Strong clinical judgment and critical thinking skills.
• Excellent interpersonal, communication, and collaboration skills.
• Ability to work effectively within an interdisciplinary team environment.
• Strong organizational skills with the ability to manage multiple priorities and deadlines.
• Proficiency with electronic health records and healthcare documentation systems.
• Valid driver's license, reliable transportation, and active auto insurance in accordance with state requirements.

Responsibilities

• Conduct comprehensive in‑home assessments to evaluate care needs and safety risk.
• Perform home safety evaluations and provide recommendations to promote independence.
• Assess participants' ability to safely perform tasks and use adaptive equipment.
• Collaborate with participants, caregivers, and family to identify care needs and gaps.
• Participate as a member of the IDT in care planning and updates.
• Coordinate with clinical and non‑clinical team members to ensure comprehensive understanding of needs.
• Monitor authorizations and renewals to prevent interruptions in service.
• Maintain accurate documentation in electronic health records and systems.
• Support quality improvement initiatives and compliance activities.
• Perform other related duties as assigned.
