Job summary

BoldAge PACE is seeking a Registered Nurse Care Manager to assess and coordinate nursing care for participants across clinic, center, and home settings in New Jersey. You will collaborate with the IDT to develop comprehensive care plans and delegate tasks to aides as needed.

The role requires RN licensure in NJ, BLS certification, and experience with frail or elderly populations; home care or managed care background is preferred.

Qualifications

• RN license required per NJ Board of Nursing.
• BSN preferred.
• BLS certification within 90 days of employment.
• Minimum 1 year experience with frail/elderly populations preferred.
• Experience in home care/long-term care/managed care preferred.
• One year RN experience required.

Responsibilities

• Provide high‑quality nursing care as part of the PACE IDT.
• Assess care needs and develop plans of care.
• Coordinate home care services and communicate with providers.
• Participate in 24/7 on-call triage for participant needs.
• Monitor acute and chronic conditions and adjust care plans.
• Reconcile MARs and ensure medication adherence.
• Educate participants and families on self-care and safety.
