Qualifications

• Requires RN license in state of practice.
• 2+ years in health care with care management experience.
• Strong communication and time-management skills.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members and care networks to meet goals.
• Coordinate and document care plan evaluations and outcomes.
