Job summary

Molina Healthcare, Inc. is hiring a registered nurse to support care management and LTSS activities across multiple Ohio counties.

You will assess members, develop and monitor care plans, coordinate with an interdisciplinary team, and help clients navigate waiver services to improve outcomes and contain costs. Travel up to 40% may be required within designated counties; you will educate members, document interventions, and ensure timely authorizations and service delivery across behavioral

Qualifications

• Minimum 2 years of health-care experience with at least 1 year in care management or LTSS.
• Active and unrestricted RN license in state of practice.
• Driver’s license and reliable transportation for travel.
• Ability to work independently with minimal supervision.
• Excellent verbal and written communication skills.
• Proficient in Microsoft Office.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and other health professionals.
• Monitor care plans and adjust as needed to meet goals.
• Coordinate services across behavioral health and LTSS to ensure continuity of care.
• Assess medical necessity and authorize waiver services.
• Lead ICT meetings for service approvals.
• Educate and motivate members using motivational interviewing and Molina guidelines.
• Identify barriers to care and provide care coordination.
• Travel 25-40% as required.
