Job summary

Molina Healthcare, Inc. is seeking an experienced RN Care Manager to support care management and LTSS coordination for high-need members across multiple counties in Ohio.

You will collaborate with a multidisciplinary team to develop and implement care plans, monitor outcomes, and promote integrated services that improve quality and reduce costs. Qualifications

• At least 2 years of health care experience with care management/ LTSS exposure.
• Active RN license in state of practice.
• Bachelor's degree may be required in some states.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care network.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of LTSS, behavioral health, and home/community resources.
• Assesses medical necessity and authorizes waiver services.
• Facilitates ICT meetings for approval/denial of services.
• Educates and motivates members using motivational interviewing.
• Identifies barriers to care and coordinates resources to address them.
• May conduct medication reconciliation as needed.
• 25-40% estimated local travel may be required.
