Job summary

Somatus is seeking an RN Care Manager to support high-need CKD/ESKD populations through in-home and clinical visits. You will partner with nephrology and PCP teams to plan care, educate patients, and coordinate services across settings.

This role requires RN licensure, multi-disciplinary collaboration, and regular regional travel to partner sites, clinics, and member homes. A BSN or CCM is preferred, with MS Office proficiency.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and physicians.
• Valid BLS certification from a licensed AHA or American Red Cross training facility.
• Renal, CKD or Dialysis care experience as main focus of your job.
• Residence in a location with high-speed internet or ability to leverage existing service.
• Comfortable traveling to partner hospitals, clinics, and community facilities within your region.

Responsibilities

• Perform home, clinic, and facility visits to support care coordination.
• Develop individualized care plans with nephrology and PCP teams.
• Educate patients on discharge requirements and renal condition.
• Lead interdisciplinary care team meetings with partnered practices.
• Establish and maintain positive relationships with providers and patients.
• Coordinate transitions of care to reduce readmissions.
• Supervise non-licensed and licensed team members as needed.
• Travel within the assigned region to conduct visits and care planning.
