Responsabilidades

• Assists care management staff with everyday duties and discharge planning.
• Collaborates with Care Managers and Social Workers, handles faxing, mail, and phone messages.
• Keeps care management updated on approvals and denials from Ensemble.
• Assists with non-complex discharge planning and transitions under supervision.
• Keeps community resource manuals up to date.
