Qualifications

• BSN preferred; RN with strong care coordination background acceptable.
• Active RN license in state; BLS required.

Responsibilities

• Identify and prioritize patients needing care management using holistic assessment.
• Document plans, ensure clear handovers at care transitions.
• Coordinate post-acute care and community resources.
• Follow discharge planning processes and continuity of care.
• Promote timely testing, treatment and appropriate resource use.
