Qualifications

• BSN preferred for BSMH or required for RSFH.
• Active RN license in state or compact licensure.
• ACM/ CCM/ ANCC certification preferred.

Responsibilities

• Identify and prioritize patients needing care management using a holistic view.
• Collaborate with the care team to plan and coordinate transitions.
• Document plans and handovers clearly in patient records.
• Promote resources and ensure timely post-acute care planning.
• Support discharge rounds and address barriers to progression of care.
• Encourage proactive care and appropriate use of resources.
