Job summary

Bon Secours is seeking a RN Care Manager for St. Francis Downtown to coordinate patient care across transitions, ensuring safe and timely care. You will work with a multidisciplinary team to assess needs, plan care, and monitor progress, emphasizing patient and family involvement.

Qualifications

• BSN preferred for role; RN licensure valid in state of practice.
• BLS Basic Life Support certification required.
• RN license in the state of work or a compact license required.
• ACM/CCM/ANCC certification preferred.

Responsibilities

• Identifies and prioritizes patients in need of care management using holistic factors and plans with patient and care team.
• Completes all documentation in patient records with clear, timely updates.
• Ensures plan of care reflects post-hospital needs and patient/caregiver involvement.
• Follows standardized practices for Advance Care Planning, LOS management, and readmission prevention.
• Supports denial prevention by addressing barriers to care progression and participating in discharge rounds.
• Promotes proactive care, removing barriers to timely testing and treatment.
