Job summary

Mercy Health is seeking an RN Care Manager to coordinate care and support safe, timely transitions across the continuum. You will screen, assess, plan, implement and evaluate care with a focus on social determinants influencing wellbeing.

Key responsibilities include identifying high-need patients, collaborating with the care team, documenting plans of care, and promoting evidence-based discharge processes. 1 year in clinical setting and BSN preferred.

Qualifications

• RN license in the state of practice (or compact).
• Basic Life Support (BLS) certification required.
• Accredited Case Manager Certification (ACM) or CCM preferred.

Responsibilities

• Identifies and prioritizes patients in need of care management using biopsychosocial factors.
• Plans with patient, caregivers and healthcare team to maximize outcomes.
• Maintains clear, timely documentation in the patient record.
