Minimum 1 year of clinical experience is required; 3 years in acute care is preferred. Strong knowledge of care coordination and reimbursement practices is essential for success.

Qualifications

• 1 year of experience in clinical setting (required)
• 3 years of experience in an acute care clinical setting (preferred)
• Ambulatory or post-acute care coordination experience (preferred)
• Bachelor of Science in Nursing preferred/required depending on site

Responsibilities

• Identifies and prioritizes patients in need of care management services using a holistic approach.
• Completes all necessary documentation with clear, concise patient records.
• Documents plan of care addressing post-hospital needs and transitional care.
• Follows standardized practices for Advance Care Planning and discharge planning.
• Supports and promotes proactive care to remove barriers to timely testing and treatment.
