Job summary

WellMed Paisano in El Paso, TX is seeking a RN Healthcare Manager to support high-risk patients and coordinate care with multidisciplinary teams. This role involves educating patients, managing transitions from inpatient to outpatient care, and ensuring a smooth care plan.

The position requires an RN with a current Texas license, BLS certification, and experience in clinical or hospital settings; bilingual English/Spanish preferred.

Qualifications

• Bachelor’s or Associate’s nursing degree with 4+ years of experience.
• Active, unrestricted RN license in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician’s office, clinical or hospital setting.
• Knowledge of COPD/asthma, diabetes, CHF and IHD.
• Proficient with electronic medical records.
• Strong interpersonal, organizational and prioritization skills.
• Ability to interact productively with multidisciplinary teams.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care with assessment, medication reconciliation, MI and goal setting, patient education, referrals.
• Assist transition from inpatient to outpatient care with assessments, contingency plans, education and post-discharge support.
• Coordinate with providers to establish or update plan of care.
• Document accurately in the electronic medical record and participate in PCC meetings.
• Prepare reports for weekly meetings and maintain evidence-based practice knowledge.
• Perform other related duties as assigned.
