Job summary

WellMed, part of the Optum family, is seeking an RN Healthcare Manager in El Paso, TX to support high-risk patients and coordinate care across multidisciplinary teams.

The role emphasizes patient education, advocacy, and efficient use of electronic medical records within a self-directed environment. A strong nursing background and BLS are required, with experience in chronic disease management preferred.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience.
• Active, unrestricted RN license in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical or hospital setting.
• Knowledge of COPD/asthma, diabetes, CHF and IHD.
• Proficient with electronic medical records.
• Strong verbal and written communication and interpersonal skills.

Responsibilities

• Identify patients at high risk with providers and clinic staff.
• Support longitudinal care for chronic conditions with assessments and education.
• Coordinate transitions from inpatient to outpatient settings and plan follow‑up.
• Develop and review plan of care with multidisciplinary teams.
• Document accurately in the EMR and produce required reports.
• Participate in daily huddles and care coordination meetings.
• Maintain current nursing practice and evidence-based knowledge.
