Job summary

WellMed in El Paso, TX is seeking an RN Healthcare Manager to support high-risk patients and navigate the healthcare system.

You will collaborate with physicians, nurses, pharmacists, social workers and other educators to empower patients and improve quality of life.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience.
• Registered Nurse with an active and unrestricted license to practice in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical or hospital setting.
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD.
• Proficient computer skills to work efficiently with electronic medical records.
• Verbal and written communication skills.
• Strong interpersonal and collaborative abilities with multidisciplinary teams.
• Organizational and prioritization skills.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care including health condition assessment, medication reconciliation, motivational interviewing, education, and referrals.
• Assist transitions from inpatient to outpatient with contingency planning and post-discharge support.
• Coordinate care plans with providers and maintain EMR documentation.
• Participate in daily huddles and PCC meetings; prepare reports for weekly meetings.
• Maintain current nursing practice competencies and evidence-based knowledge.
