Job summary

WellMed, part of the Optum family, is seeking an RN Healthcare Manager for Central Brownsville. This role supports high-risk patients to navigate the healthcare system and collaborates with multidisciplinary teams to improve quality of life.

You will conduct assessments, coordinate care plans, perform medication reconciliation, and educate patients and families. A BSN or ASN with RN licensure, BLS, and 2+ years in clinical settings are required; bilingual English/Spanish is a plus.

Qualifications

• RN license with active, unrestricted practice in state of employment.
• Current BLS certification.
• 2+ years of experience in a physician’s office, clinical or hospital setting.
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD).
• Proficient computer skills with electronic medical records.
• Strong organizational, prioritization and interpersonal skills.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care: assess health conditions, reconcile medications, set self-management goals, provide education, coordinate referrals.
• Manage transition from inpatient to outpatient: assess needs, reconcile medications, educate patients, assist with post-discharge needs.
• Coordinate with providers to update care plans and document in the EMR.
• Participate in PCC meetings and prepare weekly reports.
