Job summary

Somatus is seeking an RN Care Manager to focus on high-need CKD and ESKD patients, coordinating care across nephrology practices and patient homes. You will establish trust with physicians and implement individualized care plans while traveling within the assigned region.

The role requires RN licensure, 2+ years of RN experience with multidisciplinary teams, and strong communication skills. Travel is a core component of this field-based position, with a strong emphasis on improving patient

Qualifications

• RN license in good standing; ability to obtain licensure in required states.
• 2+ years of RN experience, including work with multi-disciplinary teams and physicians.
• Valid BLS certification from a licensed facility.
• Renal, CKD or dialysis care experience as a primary focus.
• Reside in an area with high-speed internet or can access reliable service.
• Comfortable traveling within the assigned region to partner hospitals, clinics, and community facilities.

Responsibilities

• Partner with physicians and practice staff to establish collaborative relationships focused on improving patient outcomes.
• Coordinate regular interdisciplinary care team meetings with partnered practices.
• Build positive relationships with patients and provider offices through in-person and telephonic engagement.
• Develop partnerships with provider teams to support clinical and operational goals.
• Collaborate with provider practices to optimize workflows aligning with care team processes.
• Educate provider practices on the Somatus program and integrated workflows.
• Provide complete quality care through assessments, transitions, and ongoing education.
• Travel to member homes, facilities, and physician offices for visits and care planning.
• Use nursing assessment to identify medical, behavioral, and social barriers to treatment.
• Develop individualized care plans with nephrology and PCP teams to improve health outcomes.
• Manage transitions of care to minimize readmissions.
• Assess patient knowledge of discharge requirements and renal condition; provide education and self-management support.
• Offer clinical guidance to non-licensed and licensed team members; delegate tasks as appropriate.
• Perform other duties as assigned.
