Job summary

Somatus is hiring an RN Care Manager focused on high-needs CKD/ESKD patients. You will travel to member homes, clinics, and offices to support care coordination, education, and integrated care planning with physicians and the interdisciplinary team.

Join a physician-practice partnership, drive outcomes, and collaborate with provider practices to optimize workflows and patient care in a field-based role.

Qualifications

• RN license and ability to license in other states as needed.
• 2+ years of RN experience in a multi-disciplinary team and with physicians.
• BLS certification from a licensed AHA or Red Cross facility.
• Renal, CKD or dialysis care experience as a main focus.
• Willingness to travel regionally to partner hospitals, clinics and facilities.

Responsibilities

• Perform outreach and schedule face-to-face visits with members at homes, dialysis clinics, or physician offices.
• Develop and implement individualized care plans with nephrologists and care team.
• Coordinate and facilitate regular interdisciplinary care team meetings with partner practices.
• Provide education on discharge requirements, renal condition, and self-management support.
• Build trusted relationships with provider practices to improve patient outcomes.
